Through the practice audit booking form, we collect: practice name, contact name, role, email address, phone number, approximate patient list size, and referral source. This data is used solely to prepare for and conduct the practice audit call.

4. Data Retention

Audit lead data is retained for 12 months from submission. If you become a client, your data is retained for the duration of the service agreement plus 12 months.
